There are many problems and issues associated with using WH-1000XM3 as a wireless Bluetooth headset paired with a phone or Windows 10 computer, and they can be found easily all over the internet, scattered across various forums, subreddits, and support pages. This post identifies the single, essential cause of a great many of these issues. I will reference the cause that I believe I've identified out for why WH-1000XM3 exhibits different and changing functionalities when using the wireless Bluetooth function to pair with a phone or Windows 10 computer. You can see the logic of my theory for itself in the post below, but more importantly, I link to real-world examples of various problems experienced by users, and show how those problems are just various manifestations of this single, essential problem that this post identifies (though surely not for the first time) with WH-1000XM3: the inability to process the two wireless Bluetooth profiles it is equipped with, A2DP and HSP, simultaneously, which in turn prevents us, the users, from enjoying the two main features of WH-1000XM3 - high-fidelity wireless audio, and two-way wireless voice communication - at the same time.

Section I: Summary of the Problem
Here is a good summary of the main issue at hand that I found in a reply to this post in the Superuser Stack Exchange Q&A site. Briefly, WH-1000XM3 installs three (technically 2, see Section II for details) devices when you pair it with a Windows 10 computer. The problem comes from an inability to use the two main functions of WH-1000XM3 - high-quality wireless audio, and 2-way voice communication - at the same time on Windows 10:
• The "Stereo" device is the main high-quality playback device of the headset, while the "hands-free" device is a lower quality monophonic playback device
• Attempting to capture audio from the Bluetooth microphone (which is also labeled "hands-free") disables the main playback device, but not the hands-free playback device.
• By having one application play audio from one playback device and another application playing from the other, the hands-free device also disables the main device.

Section II: WH-1000XM3 & Windows 10
When you initially pair a WH-1000XM3 with a computer running Windows 10, not one but THREE new audio devices will appear in the Windows "Sound" settings dialogue box (NOT the "Sound" tab in the new Windows 10 Settings app; it's absolutely insane that Windows 10 has two different settings windows with the exact same name, but that's for another post).

These are the three devices that will appear when you pair WH-1000XM3 with Windows 10:

However, as mentioned previously, these "three" devices actually fall into two groups, based on which Bluetooth "profile" they are associated with (more on these profiles in the next section). The "Headphones" device, with the "WH-1000XM3 Stereo" label (#2 in the picture above), and the two "Headset" devices, both with the "WH-1000XM3 Hands-Free AG Audio" label (#1 and #3 above). The two Headset devices, numbers 1 and 3, are two sides of the same coin, with that coin being one Bluetooth profile; the Headphones device, number 2, is a separate coin, i.e. a separate Bluetooth profile. So why are the two sides of the Headset coin displayed separately, one in the "Playback" tab and one in the "Recording" tab? In order to understand this, you need to understand Bluetooth profiles.

Section III: Bluetooth Profiles
Why do the WH-1000XM3 headphones install 3 sound devices when you pair them with a Windows 10 computer? Because the WH-1000XM3, along with seemingly all other high-end wireless Bluetooth headphones with headset functionality, such as the Bose QuietComfort 35 I and QC 35 II products, make use of two separate Bluetooth profiles:
So, what is a Bluetooth profile? Wikipedia says it is a "specification regarding an aspect of Bluetooth-based wireless communication between devices." In my understanding of Bluetooth profiles, they are somewhat analogous to the different internet protocols, which are like specifications regarding different aspects of communications over the internet, rather than Bluetooth. The A2DP profile is analogous to the FTP protocol: used to quickly transfer lots of data (more data is analogous to higher quality audio) in one direction (from the server to the client). The HSP profile is analogous to the old IRC protocol, which was used for text chatting between internet users: used to transfer small amounts of data (i.e., low-quality audio, in this analogy) in two (or more) directions (between users).

Section IV: The Essential Problem with WH-1000XM3
Real-world examples of various end-user problems caused by this essential problem are documented throughout.
So to backtrack for a second: Sony WH-1000XM3, Bose QC 35 II, and I assume any similar pair of high-end, wireless Bluetooth headphones with voice communication functionality, all are built to make use of both of these profiles, A2DP and HSP. The problem appears to be that the headphones themselves, not Windows, the audio drivers, the Bluetooth drivers, or the Windows Bluetooth settings, are NOT ABLE TO USE BOTH BLUETOOTH PROFILES SIMULTANEOUSLY. I do not know why this is; WH-1000XM3 simply is not able to process two streams of data specified by different Bluetooth profiles simultaneously, preventing users from enjoying the two primary functions of WH-1000XM3 at the same time, and having to switch back and forth between them, which is made an arduous process by the fact that only one profile will let your audio sound good, and only the other profile will let you use the microphone on WH-1000XM3, and you cannot use both profiles at the same time. So, would you like to listen to music in the background while you chat with your college roommate over Skype? Well, Skype needs the HSP profile, so your music is going to sound like garbage!
Or, would you like to be able to hear the sounds in a video game that you are playing, while you communicate with your teammates over Discord? Sorry! The video game developer wanted you to have high-quality audio so their game requires 2-channel, 16-bit, 48000Hz sound processing, but Discord needs to listen to the microphone on WH-1000XM3, so you're out of luck, Windows is going to force the HSP profile because an application is listening to the mic, so there goes your in-game audio! (And another example of this same user experience).
If you don't necessarily believe me, it is actually very easy to observe your headphones switching between the two Bluetooth profiles, when it is paired with an iPhone (I assume this works with any Bluetooth phone, but I've only tried it with an iPhone). Pair your WH-1000XM3 with an iPhone, and play some music. The audio quality is superb, as it should be on a $300 pair of headphones. Then, while you're listening to the music, have someone call you. As soon as your phone detects an incoming call, it switches your headphones from the high-quality A2DP profile, which is for listening to music (high-quality, only works one-way), to the HSP profile, which is for talking on the phone (lower-quality, lets you send and receive/play audio simultaneously). As your music pauses and the phone starts to ring, you will hear a dramatic, obvious, unmistakable drop in audio quality. Try hanging up the call, and listen to the change in quality as the profile switches back to A2DP from HSP - there is a dramatic increase. Thus, your iPhone switches between the two profiles, A2DP and HSP, depending on what you are using your headphones for. Listening to music -> high-quality one-way audio -> A2DP profile; talking on the phone -> lower-quality two-way audio -> HSP profile.
Apple literally has an entire support page that implicitly acknowledges this essential problem with current wireless Bluetooth technology as implemented in high-end wireless Bluetooth headsets like WH-1000XM3: "If you listen to music or other audio via Bluetooth headphones, and then you open an app that uses the Bluetooth headphones' built-in microphone, the audio quality and volume are reduced." This is exactly one of the outcomes explained below.
And guess what that support page recommends? That's right, "Close apps that use the microphone."
So, how does this manifest on Windows 10, as opposed to an iPhone? You can find the answer all the way in Section I of this post, in the quote from the reply to a forum post. Here it is again:
• The "Stereo" device is the main high-quality playback device of the headset, while the "hands-free" device is a lower quality monophonic playback device
• Attempting to capture audio from the Bluetooth microphone (which is also labeled "hands-free") disables the main playback device, but not the hands-free playback device.

So on both Windows and iPhone, WH-1000XM3 is ONLY ABLE TO USE PROFILE OR THE OTHER AT A TIME. Here is another way to explain how Windows handles these two profiles:
  • If you disable the Headset devices (#1 & #3 from Section II) and enable the Headphones device (#2), Windows will use the A2DP profile
    • Audio quality will be high across all Windows applications
    • Audio will only travel one-way, from Windows to WH-1000XM3 (no voice communication possible)

  • If you enable the Headset devices and disable the Headphones device, Windows will use the HSP profile
    • Audio quality will be low across all Windows applications
    • Audio will travel two-ways, between Windows & WH-1000XM3 (voice communication possible!)

  • If you enable all three devices, you have the option to select the devices on the Windows 10 Sound control panel:

With these settings, Windows will hot-swap between A2DP and HSP profiles
With all 3 devices enabled, the following combinations of input & output selections produce the listed outcome:
  • If you choose Headphones (WH-1000XM3 Stereo) as your output device and and Headset (WH-1000XM3) as your input device, Windows will hot-swap between A2DP & HSP like iPhone does (although unlike the iPhone, Windows doesn't know to pause your high-quality audio when switching to HSP, you can observe this by playing a song on iTunes, and then recording a voice memo)

Section V: Conclusion
WH-1000XM3 can only process either the A2DP wireless Bluetooth profile, or the HSP profile, but not both simultaneously. On Windows, WH-1000XM3 represents these two "modes" as the "Headphones (WH-1000XM3 Stereo)" device and the two "Headset (WH-1000XM3 Hands-Free AG Audio)" devices. The Headphones (WH-1000XM3 Stereo) device is associated with the A2DP profile, and the Headset (WH-1000XM3 Hands-Free AG Audio) device is associated with the HSP profile. Because WH-1000XM3 can only process one or the other profile at a time, when both devices are enabled in the Sound settings panel...

...Windows will exhibit one of two behaviors, depending on yet more variables that, while they do affect how Windows behaves with respect to WH-1000XM3, I think are beyond the scope of this post, as they do not change the ultimate problem we are left with, which is the inability of WH-1000XM3 to process the two Bluetooth profiles simultaneously, which in turn prevents us, the users, from enjoying the two main features of WH-1000XM3 - high-fidelity wireless audio, and two-way wireless voice communication - at the same time.
Mobile phone manufacturers have incorporated this "defect" in WH-1000XM3 into their products rather seamlessly - using the high-quality A2DP profile to process music, videos, and other applications that require only one-way audio, and pausing those applications in order to switch to the lower-quality, two-way HSP profile when necessary, such as to process a phone conversation, which requires two-way audio. Alas, Microsoft is not so smart.
Windows will usually, but NOT ALWAYS, default to the A2DP profile when no applications are listening to the headset (microphone) on WH-1000XM3, and then instantly switch to HSP, apparently without notifying any applications that were using A2DP while it was active, because none of them react to the switch, such as by pausing a song that was playing. Users will experience this as ANY AUDIO THAT WAS PLAYING OVER A2DP SIMPLY CUTTING OUT WHEN IT IS DISABLED IN FAVOR OF HSP. Not a very positive user-experience, Sony!
And worse, under certain circumstances, including enabling and disabling the Headset and Headphone devices and setting various of those three devices as the "default device" and "default communications device" in the above settings panel, in specific combinations/arrays, can cause windows to default to the HSP profile. The user experiences this as ALL WINDOWS AUDIO BEING PROCESSED VIA THE HSP PROFILE, RESULTING IN UNIVERSALLY TERRIBLY AUDIO QUALITY.
